EduTool Access Request Form (faculty)

Please fill out this form to request the use of one of our supported EduTools for teaching your hybrid or online course.

This form is for faculty use only. If you are a student, please contact your instructor for directions on accessing the EduTools utilized in your course.

While your access to these EduTools can be continued for future semesters, we ask that you notify us each semester of your intentions to use each tool so that we can better manage our resources.

If you plan to use multiple EduTools, please complete this form once for each. If you’re interested in using RealizeIt to create an adaptive course, you will need to complete the RealizeIt Course Application and will need to submit an endorsement form signed by your department head.
